If you are a full-time team member (working an average of 30 or more hours per week), you will be eligible for benefits the first of the month following 30 days of full-time employment. Your Human Resources representative will be contact you when your eligibility date approaches.

Company Paid and Voluntary Life Insurance

Company Paid Basic Life & AD&D Insurance

All full-time team members will receive 1x their annual salary rounded up to the thousand in company paid life insurance at no cost! The benefit carries a minimum of $30,000 and maximum of $200,000 in coverage. Benefit includes reduction of coverage after the age of 65, see Certificate of Coverage for full details.

Voluntary Life Insurance & Accidental Death & Dismemberment (AD&D)

Team members may request voluntary life insurance for themselves, their spouse, and eligible dependents (up to age 26).  For those Team Members that are newly eligible for benefits, you are offered coverage up to $150,000 for yourself, and $30,000 for spouse as guaranteed coverage without having to provide evidence of insurability (EOI).

For requests above the guaranteed amounts, EOI form is needed and coverage is subject to underwriting by New York Life.

  • Employees – Must elect in $10,000 increments, but not to exceed 5x your salary (or $500,000 max)

Note: Team Members who are age 70 or older; maximum benefit is $50,000

  • Spouse – Employees must elect Voluntary coverage for themselves to add coverage for spouse. Must elect in $5,000 increments, coverage can’t exceed 2.5x team members annual salary & coverage amount not to exceed 50% of the Team Member’s elected coverage for Voluntary Life insurance up to a max of $150,000.
  • Dependent – Birth to 6 months: $500 max; 6 months to age 26: $10,000 max

Note: coverage for dependents does not require EOI.  You pay one rate for all eligible dependents.

Outside of new benefit eligibility (during open enrollment periods), any requests for voluntary life insurance for Team Members and spouses will require the EOI form to be completed & to be approved by New York Life.

Short & Long Term Disability

Short Term Disability

Short Term Disability insurance is designed to protect an employee’s income for a short period of time in the event an employee becomes ill or injured and is unable to work up to 13 weeks. Benefit amount – 60% of your weekly salary for up to 13 weeks (following an 8 day waiting period). See full plan description for specifications and limitations.

Long Term Disability

Long Term Disability insurance is designed to protect an employee’s income for up to five years after the short-term disability period has expired (at least 90 days from event).

    • Benefit amount – 60% of your salary for up to 5 years following 24 months of coverage. See full plan description for specifications and limitations.
    • Employees who are first time eligible for this coverage do not need to provide evidence of insurability. Employees who waive the coverage when they are first time eligible will need to provide evidence of insurability if they wish to elect coverage later (during a subsequent open enrollment).
    • Managers level and above receive this benefit free of cost.

Health Savings Account (HSA)

HSA – Health Savings Account

What is a HSA – Team Members who enroll in a High Deductible Health Plan (HDHP) qualifies to open an HSA (Health Savings Account).  You can use pre-tax funds, up to $4,150 ($8,300 family coverage) annually, to be deposited into an account to pay for out-of-pocket, eligible healthcare expenses. Unspent money is portable and rolls over into the next benefit year to cover expenses or to continue earning interest tax free.  HSAs also differ in that you can take your money with you if you switch employers, or even if you change health insurers.

As you add funds into your account, your contributions reduce your annual taxable income.  The funds you withdraw to pay for qualified medical expenses remain untaxed and you save again when funds in your HSA accounts earn interest tax free.

Flexible Spending Accounts (FSA)

Health Care-FSA

Team members may participate in the medical reimbursement account and set aside up to $3,050 in 2023 and up to $3,200 in 2024 a pre-tax  dollars that can be used for medical, dental, vision and other eligible expenses as outlined by the IRS for you and your eligible dependents. Funds are eligible for use as of the eligibility date, contributions will be deducted pre-tax thought the plan year from your paychecks. You may transfer up to $610 in 2023 and up to $640 in 2024 unused dollars into the following plan year.

Dependent Care Account

Team members may participate in the dependent care account and set aside up to $5,000 in pre-tax dollars to help pay for childcare. Funds are eligible for use once every payday. This plan is a use it or lose it benefit.

Sun Health Wellness – Healthy Mindsets Portal 

Employee Association

The Employee Association is a nonprofit organization that was formed in 2008 in an effort to aid Sun Health team members in times of need. The concept of the organization began when a team member approached management with a request to hold a bake sale to assist with fellow team members’ medical bills. With more than 400 team members on staff at that time, leaders saw a tremendous opportunity to create a formal plan to support our team members in need. As a result, the Employee Association was formed.

Over the years, the Association has brought relief and support to many team members in need and is sure to positively impact the lives of many more to come. The Employee Association is just one of the ways Sun Health shows its gratitude for our team members’ commitment to our mission to champion healthy living, research and superior health care.

Purpose

This association was created to provide monetary assistance to the members of the Sun Health Employee Association for circumstances such as the occurrence of any unexpected event or combination of circumstances that jeopardize the individual’s health and welfare.
Example of undue hardships include:

  • Offering assistance in time of need e. g. funeral expenses for the death of an immediate family member or the travel costs to see an ailing close relative.
  • Living expenses due to damage caused to team members dwelling
    (fire or storm)
  • Significant loss of household income due to events such as robbery or emergency medical expenses.
